We can help with pain and symptom management, emotional and psychological support, and can discuss advanced care planning or preferred priority care. A patient who is a carer. We offer support and practical advice to people who are looking after a friend or relative who has a life limiting illness. We can help with 1:1 therapy, counselling, benefits advice, social work advice and spiritual support. Download our pack for further details of the services we provide.
Making a referral

End of Life Partnership
St Luke’s Hospice is committed to improving end of life care across Cheshire, and with our passion for education and changing attitudes we hope to contribute to change across the UK. Our involvement with the End of Life Partnership has led to us being recognised nationally and in November 2015 we received the award for Best Training Initiative at the National Dementia Care Awards.
