By completing and signing the form below, I confirm that:
I wish St Luke’s Hospice Trading Ltd to attempt sell goods on my behalf, in any manner it decides, to maximise the value of the sale. 5% (plus VAT) of the sale will be paid by way of commission to St Luke’s Hospice Trading, who will reclaim 25p tax on every £1 that I have given.
I own all items donated for sale and that they were obtained in a legal manner.
It is not necessary for St Luke’s to write to me to confirm the donations, provided that the total value of the net sale proceeds in that tax year has not exceeded £1000. Once a sale is made that brings the total value of the net proceeds of sale of goods to more than £1000 St Luke’s Hospice Trading must first write to me to confirm my consent to those funds over and above the initial £1000 being donated.
I am a UK taxpayer and I agree to St Luke’s (Cheshire) Hospice claiming tax on all cash donations I have made for 4 years past, present and future until I notify you otherwise as gift aid donations.
I confirm that if I pay Income tax and/or Capital Gains Tax for the current tax year (6 April to 5 April), less than or equal to the total amount all charities and Community Amateur Sports Clubs (CASCs) I donate to reclaim on my gifts, it is my responsibility to pay any difference. I understand other taxes e.g. VAT or Council Tax do not qualify.
I will notify St Luke’s Trading Ltd by written notice of any change to my details or if I cease being a tax payer.
St Luke’s Trading will write to me at the end of the tax year regarding your gift aid donations, or if they vary the terms of this agreement or cancel my gift aid declaration at any time.
